Please arrive 15 minutes early from your appointment time to fill out necessary paperwork. Please bring comfortable clothing like you would going to a gym to exercise and wear tennis shoes. The Physical Therapist will evaluate you and perform some treatment depending on time available as well as develop a Home Exercise Program to start off with. Please call the nearest location if you would like to fill out the contact form online.
As of September 1st, 2019, Patients who are paying through self-pay/private pay as well as certain insurances, patients do not need a referral from a physician to be treated by any of our therapists for the first 10-15 business days from their initial evaluation. Once those days have been met, a patient would need a referral to continue with services. If you have insurance and are planning to use your insurance (For example: Medicare), a referral will be required for an Initial Evaluation & Treatment Visits after your Initial Evaluation Visit.
Currently, we are enrolled in Medicare, BCBS, Humana, Tricare East, United Health, Aetna, and IMS insurance and we are considered an out-of-network provider for all other insurance companies at this time.
If you have out-of-network benefits with your private health insurance, we will provide you with all the documentation necessary to turn into the health insurance company for reimbursement.
Payment for our services is due on or before the service date to protect our patient slots and avoid missed visits or cancellations.

For In House/House Call Outpatient Services: The initial evaluation can take up anywhere from 30-60 minutes depending on a variety of factors because every patient is different. The following treatment sessions can take 30-60 minutes each.
For Online/Telehealth Outpatient Services: The initial evaluation and following treatment sessions take about 30 minutes each depending on a variety of factors because every patient is different.
For Traditional Home Health Visits: The initial evaluation and following treatment sessions take about 30 minutes each depending on a variety of factors because every patient is different.
For the Clinic: The initial evaluation can take up anywhere from 30-60 minutes depending on a variety of factors because every patient is different. The following treatment sessions can take anywhere from 30-60 minutes each.
Although your Therapist will be able to provide you with a much more accurate timeline following your initial assessment, as a general rule of thumb, the following holds true: For minor movement challenges or non-chronic injuries, we usually see dramatic improvement within 4-6 sessions. For more major movement issues or chronic injuries, we ballpark around 10-12 sessions. Finally, for severe or long-term chronic issues, it may take up to 20 sessions to see a significant resolution of your symptoms. These are very general estimates and every patient case varies, and often you’ll feel improvement from the very first session. Your therapist will work with you to create a program that will be tailored to your needs during your initial session, so you’ll have an idea of your roadmap once you get seen.
For House Call visits, the physical therapy session will require a quiet and private space. You will need enough room for a folding treatment table.
Outpatient House Call visits would be for those who need the convenience of a therapist going to their home to perform their therapy. Whereas, traditional Home Health Services would be for those who are considered home-bound due to weakness or other medical conditions making it difficult for a patient to leave their house.
A massage table, massage tools, exercise bands, and small weights as necessary.
Simply put, telehealth is the use of electronic information and telecommunications to promote long-distance health care. This can include video conferencing, streaming media, wireless communications, and other forms of internet communications. For many patients, work, family, or travel constraints preclude them from attending therapy three times per week. Telehealth enables a therapist to remotely monitor a patient’s Home Exercise Program and updates notes, without the patient needing to drive to the clinic. It’s a win-win situation and a patient can save time and be seen by a preferred clinician.
You will need a computer or laptop with a webcam and microphone. It is preferred that you have headphones to hear each other better; however, it is not needed. Make sure you have a strong connection for better video quality and it is preferred that you are in a area where there is space to do exercises.
